Specialist Reablement Services

Sevacare has vast experience of delivering high quality Reablement Services, which help people ‘do things for themselves’, rather than ‘having things done for them’. Our approach to reablement is an active process of regaining skills, confidence and independence. Sevacare provides reablement to a number of Service Users, not just older adults, following an acute medical episode or to reduce the gradual decline of Service Users independence. A reablement service is a short-term intensive input, which aims to reduce the need for further care and support, increasing the Service User’s independence and control over their life.

Sevacare believes a high quality reablement service should be as follows:

 Reablement
  • Supporting people to do things for themselves, rather than doing things for them.
  • A time limited service, usually of 6-8 weeks, which is agreed from the start of the reablement service in the Service Users Reablement Support Plan.
  • An outcome focussed personalised care and support service. The end goal is to support Service Users to regain their independence in their home and the community.
  • One which sets specific goals and targets to achieve the Service User’s desired outcomes, agreed between the Service User, their Carer/family and the reablement care team.
  • Individualised, tailored care and support aimed to support the Service User’s independence at home.
  • Aiming for ‘positive change’ during at the end of the service duration.
  • Supporting Service Users in what they can already do and assisting them to regain skills, to increase their confidence and to regain independence.
  • Include an assessment stage that is dynamic not static. Sevacare believes that you cannot decide on the care and support package on a single one off assessment. We assess the Service User and observe over a defined period of time as the Service User’s needs and abilities may change.
  • Working collaboratively with the Service User, their Carer/family as well as other support from the Council’s Reablement Teams, specialist organisations and third sector groups.
  • One which embrace the use of assistive technology and Telecare solutions to assist with reablement.
